Scar Treatment

The term Scar is derived from the Greek word “ekshara” which means scab. Scab is basically a rough surface or a dry skin different from a normal one. A scar is an abnormal marking on the skin due to variety of reasons like trauma, blister, injury which causes raise or elevation on the surface of the skin. Later we shall discuss about types of scars and what is the best way to treat.

The main reason for the onset of scar or an ugly mark is an after effect of a wound or trauma skin. The injuries include the outermost layer known as epidermis as well as a subcutaneous layer which contains fat called as hypodermis and superficial dermis. The trivial injury repairs without scarring but in case of grave injury there is scarring because the layer of the dermis is affected.

Three Special Stages Of Wound Healing

The process of scarring can be understood by learning about the wound healing process.

1. Inflammation/ Swelling Stage:

In the site of injury there is a blood clot (hematoma) which prevents any further blood loss and also act as an obstruction for the micro-organisms to cause further infection. Platelets help in clotting of blood just like a leaking pipe plug. The white blood cells like neutrophils and monocytes arrive at the injury site along with the immune cells in our body to stop further growth of bacteria. The macrophage in our white blood cells stops the bacteria to deteriorate our wound by eating the micro-organisms and digests them.

2. Proliferation / Growth Stage:

This stage involves the growth of new tissue. The epithelial tissue cover the wound, deep wound requires the growth of scar tissue along with the skin tissue. The new tissue comes atop of the wound bed replacing the damaged tissue.

3. Remodeling / Maturation Stage:

The extra collagen which is formed during the growth stage are expelled and cleared away in this phase. The enzyme in our body helps to produce the necessary changes here.

Types of Scars:

They are mainly 3 types of scars which occurs frequently

1.Normal Scars :

The normal reaction of the skin to cure causes scarring. When there is injury in the dermis layer of the skin 5 to 10% of the normal skin’s effectiveness collapses and it takes 7 to 10 days to develop a scar after serious trauma. The regeneration takes about 7-12 months which consist of almost 80% of the original skin tissues. This types of scars are normal one.

Treatment for Normal Scars :

It is obvious that scarring makes changes in the color of the skin. Some coloration can be regained with time but some are permanent and cannot be treated. In some cases, skin injury creates the skin color either darker or lighter than the normal color. The state of post-inflammatory hyperpigmentation or hypo-pigmentation can be treated. The dark patches on the skin (hyperpigmentation) can be treated by prescribing hydroquinone.

2.Raised Scars :

The irregular and unusual growth of the scar tissue rather than what is needed causes raised scars. These are further classified into Keloid and Hypertrophic. This types of scars are raised one.Both look unattractive and are

Keloid Hypertropic
Keloid scar takes time to form as a complete intricate structure and is basically found on the surface of the wound It is formed soon after the skin has been treated.
These are busy scars which are hard working and multiply quickly creating pain and itchiness. It is not severe as Keloid but is often found at the sites of burn, piercing, pimple, and cut.
Treatment for Raised Scars :

As discussed in the above table the Corticosteroids are injected to minimize the process of growth. There is a fear of atrophy or the wasting of tissue. The use of fractional CO2 laser to heal raised scars is also done and in this case there is less risk of atrophy. Sometimes the Keloid scars need surgical excision, removal of tissue with a scalpel. Surgical treatment or revision surgery is performed for Hypertropic scars.

3.Intented / Atrophic Scars :

This is generally formed under the circumstances of chicken pox, acne, and various other reasons. Atrophy is the decline in the growth of tissue. It can be found in any areas in the body when there is scanty scar tissue leading to lessen fat content beneath the skin. It is quite opposite to the type B scars. This types of scars are intented one.

Treatment for Intented Scars :

The collagen packs the area to elevate it to the normal degree of the skin making it uniform and smooth. It can be done in number of ways like

1. Fractional CO2 laser

2. TCA Cross

3. Chemical peels

4. Skin Needling

5. Non-ablative laser
